Near Misses are those events that might have resulted in harm to a patient but were discovered and corrected before they ever reached the patient.

Goal

The New York Chapter of the American Collegeof Physicians (NYACP) and the New York Special Interest Group of the Association of Program Directors in Internal Medicine (APDIM-NYSIG) are working with the New York State Department of Health to collect data relevant to “near misses” in patient care in a safe, anonymous and risk-free environment. Through this web based tool, we hope to prove that voluntary, anonymous reporting is an effective way of identifying vulnerabilities in our health systems and learning about the strength of the barriers that protect patient from harm.

Confidentiality

The Near Miss Survey data is being held securely in a server belonging to the New York Chapter American College of physicians. All non-identifiable responses related to the survey are privileged as quality research information under section 206 (1) (j) of the New York State Public Health Law.
